i,m going to guess that your wrist is perhaps wobbling a bit off the graver main axis. a bit left, then right then back left & so on.. as you roll the graver (intentionally) right or left to go from wide to narrow, or vice versa, the wrist wobble can cause this. pay close attention to your hand...
